Scheduling a New Appointment

Allow about 1-1/2 to 2 hours for your first office visit. The first visit is lengthier because obtaining complete medical information about your present and past health conditions, review of your current supplements, and evaluation of your complaints from a Chinese medical perspective are all time-consuming. Depending on the complexity of your medical issues and the medical records that are available at the time of the consultation, preliminary discussion and plan of treatment as well as a limited physical exam may occur. Time will also be allowed for an initial medical acupuncture treatment, if appropriate.

Follow-up office visits usually take 30-45 minutes and almost always include medical acupuncture. We know your time is valuable, and we try to stay on schedule. If you have time considerations, please advise us when you arrive. You can help us keep a balanced schedule by letting us know at least 24 hours in advance if you must cancel or reschedule your appointment.

As a new patient, you will be responsible for obtaining your prior medical records from other physicians. These records may be faxed to (775) 827-1219 prior to your first appointment, or you may bring them with you on the day of your visit. Please use our Request for Release form (PDF).

Please hand-carry all vitamins, herbs, supplements and prescription drugs that you currently take.

Please fax or mail a completed Patient Information Form (PDF) and Health Questionnaire (PDF) at least one week prior to your appointment date. Please bring the original Form with you to the appointment.
